Mistakes to avoid on this form
- Affidavit of service (UD-3) is missing. The server must be someone other than you and over 18.
- Affidavit of service is incomplete: no description of the person served, a missing photo, service on a Sunday, or military-service questions left blank.
- Improper service outside the United States.
